We take the time to get to know you!
As an independent contractor you will have the benefit of determining your pay rate and the hours you will work with your clients. Upon registration, you will have the opportunity to meet several Lynden Nursing office team members in person. We take the time to get to know you, understand your likes, dislikes and other objective preferences, allowing us to find appropriate client referrals for you.
You have the right to pursue or decline any client opportunity we inform you about.

Self Care Health Services understands the importance of finding the right caregiver when you or your loved one needs it most.
Our office team is committed to referring qualified caregivers who can assist seniors with their daily needs. Referred caregivers can deliver support to enhance seniors’ quality of life, while keeping them safe and independent in their homes
Because we care
All referred caregivers are thoroughly screened with FBI level 2 background checks indicating eligibility. Professional licenses and certifications are also verified.
Types of Caregivers We Can Refer
HHA
Home Health Aide
A trained and certified health-care worker who assists a patient in the home with personal care (such as hygiene and exercise) and light household duties (such as meal preparation) and who monitors the patient’s condition.
CNA
Certified Nursing Assistant
A certified nursing assistant helps patients with activities of daily living and other healthcare needs.
LPN
Licensed Practical Nurse
A nurse who has completed a practical nursing program and is licensed by a state to provide routine patient care under the direction of a registered nurse or a physician.
RN
Registered Nurse
A nurse who graduated from a college’s nursing program, or from a school of nursing and has passed a national licensing exam

Caregiver Registration Criteria
Registered Nurses and Licensed Practical Nurses
- RN or LPN license
- Social Security Card
- Level 2 Background screening on file with AHCA
- Valid CPR card from American Heart Association, American Red Cross or American Health and Safety Institute
- Communicable disease statement done within the last 6 months stating “free of communicable diseases”
- 1 hour of HIV/AIDS Certificate
- 30-hour Infusion Therapy Certificate (LPN only)
- Professional Liability Insurance – recommended (Most clients require it)
- Current Florida Driver’s License or Florida I.D.
Certified Nursing Assistants and Home Health Aides
- CNA License in Florida (or HHA certificate – 40 hours minimum)
- Social Security Card
- Level 2 Background screening on file with AHCA
- Valid CPR card from American Heart Association, American Red Cross or American Health and Safety Institute
- Communicable disease statement done within the last 6 months stating “free of communicable diseases”
- 1 hour of HIV/AIDS Certificate
- Alzheimer’s Training Certificate – 1 Hour
- Self-Administration of Medication Certificate – 2 Hours
- Professional Liability Insurance – recommended (Most clients require it)
- Current Florida Driver’s License or Florida I.D.
